Immunizations & Vital Records Are Moving! 🎉

We’re excited to share that our Immunizations and Vital Records offices are moving to a new location in Saratoga Springs this February! The new space will allow us to better serve our community in a modern, welcoming environment.

Important Dates to Know

  • Friday, February 6: Last day of services in American Fork (Immunizations & Vital Records)
  • February 9–16: Closed for move:
  • Tuesday, February 17: Reopening in Saratoga Springs

During the closure (Feb 9–13), clients needing immunizations or vital records services can visit another UCHD location.

What’s Moving?

  • To help avoid confusion:
  • ✅ Moving: Immunizations and Vital Records
  • 🏠 Staying in American Fork: WIC services
  • The WIC office will remain in American Fork for the time being and is not impacted by this move.
