We’re excited to share that our Immunizations and Vital Records offices are moving to a new location in Saratoga Springs this February! The new space will allow us to better serve our community in a modern, welcoming environment.

Important Dates to Know

• **Birth/Death Certificates will open Monday, February 23rd**
  • Birth/Death Certificates Hours
    • Mondays & Wednesdays 8-11:30 AM, 1-4:30 PM [Through May 1]
    • Monday-Friday; 8-11:30 AM, 1-4:30 PM [Starting May 4]

During the closure (Feb 9–13), clients needing immunizations or vital records services can visit another UCHD location.

What’s Moving?

• 🏠 Staying in American Fork: WIC services
• The WIC office will remain in American Fork for the time being and is not impacted by this move.
