You know a lot about where you live. You make a difference in the health and well-being of Utah County! Please tell us what matters most to you by filling out this survey. Private information is not required, and your answers cannot be tied back to you. All responses will remain anonymous.

The Utah County Health Department and its community partners will use this information to find out what health issues you care about and your ideas to make our community healthier.
