What To Bring to the Immunizations Clinic
To ensure a smooth and efficient visit, please bring the following items:
Insurance Information
- Bring a current insurance card.
- Billing is provided as a courtesy; however, coverage varies by plan and cannot be guaranteed.
- Contact the insurance provider using the number on the back of the card to verify if the Utah County Health Department is considered “in network.”
- If insurance does not cover the vaccines received, payment is required at the time of service.
- If a vaccine is billed to insurance and the claim is denied, a bill will be issued for the balance.
- Travel immunizations are not billed to insurance; payment is due at the time of service.
Immunization Records
- Bring any available immunization history or records.
Form of Identification
- Acceptable forms of ID include:
- Driver License
- State ID
- Passport
- Student ID

Note: All card payments incur a 2.65% service fee, with a minimum charge of $1.50, payable by the customer.
