How Do I Find My Records?
Whether you’re requesting records for yourself or a child, there are several ways to locate vaccination history depending on where and when the vaccines were received.
Vaccines Administered at Utah County Health Department
If your vaccinations were provided at a Utah County Health Department clinic, you may request a copy of your record by completing the following form:
- The completed form can be brought to any UCHD location for same-day, in-person printing of your record.
- Alternatively, follow the instructions on the form to submit it by email or fax.
Vaccines Administered by Another Utah Provider
Vaccination records from other providers in Utah may be available in the Utah State Immunization Information System (USIIS).
However, please note:
- Not all providers submit data to USIIS.
- Records printed from the state database may be incomplete.
You may also be able to check your state immunization record using the Docket app.
Vaccines Administered in Another State or Over 10-15 Years Ago
If vaccines were received:
- In another state, or
- More than 10–15 years ago
Please contact the original provider or clinic directly to request the vaccination record.
Other Immunization Record Sources
K-12 Schools
If the individual was recently enrolled in a K–12 school, a copy of the immunization record may be available through the school registrar.
