Online Orders

Utah County Vital Records sends all vital records via First Class Mail through the United States Postal Service (USPS). Utah County Vital Records is not responsible for any lost, misdirected, or undelivered mail by USPS or any other delivery service.

In-Person Orders

Birth, stillbirth, and death certificates can be requested in person at any of the office locations. A current photo ID and a valid form of payment are required. If requesting a certificate for a relative, proof of relationship, such as a birth or marriage certificate, may also be needed.

Mail-In Orders

All requests for birth, stillbirth, and death certificates must include a completed request form, correct fees, and valid photo identification. If requesting a certificate for a relative, documentation such as a birth or marriage certificate may be required to verify the relationship.

Requests submitted without the required fee or valid ID will be returned. Please allow up to one week for processing and mailing.

If a request form cannot be printed, a written letter may be submitted. The letter must include:

  • The type of record requested (birth, stillbirth, or death)
  • Full name of the person on the record
  • Date and place of the event
  • Full names of the parents (including maiden names, if applicable)
  • The requestor’s relationship to the person on the record
  • Reason for the request
  • Requestor’s signature

Certificate Order Information

Eligibility to Request a Certificate

Certificates may be requested by the person named on the record, an immediate family member, a legal guardian, or a designated legal representative. If requesting a certificate for a relative, proof of relationship, such as a birth or marriage certificate, may be required.

Identification Requirements

Valid photo identification is required when requesting a Utah birth certificate.
For mailed requests, an enlarged and clearly visible photocopy of the front of the ID must be included. Requests submitted without acceptable identification will be returned.

Legal Warning

It is a criminal offense to make false statements on vital records request forms or to fraudulently obtain a birth certificate.
Violations may result in a civil penalty of up to $5,000 and imprisonment for up to five years.

Availability of Records

Birth Certificates:
Certified copies of Utah birth records from 1931 to the present can be requested at this office.
For birth records prior to 1931, Utah County records are available from 1905. Records from other Utah counties prior to 1931 may be requested for an additional fee.

Death Certificates:
Certified copies of Utah death records from August 2006 to the present are available at this office.
For earlier records, Utah County death records are available from 1905. Records from other counties prior to August 2006 can also be obtained here for an additional fee.

Public Access to Records

  • Birth records become public 100 years after the date of birth
  • Death records become public 50 years after the date of death
  • Marriage and divorce records become public 75 years after the date of the event

Some public records can be searched through the Utah State Archives.
