This application requests personal data to determine eligibility and enroll participants in the requested program. The information provided will be used for program administration, service delivery, and required reporting. If the requested information is not provided, eligibility may not be determined, enrollment may be denied, or participation may be discontinued. Personal data may be shared with authorized Utah County departments, state or federal program administrators, or contracted partners as required to operate and monitor the program.
