This form requests personal data to register participants, manage attendance, and communicate information related to the class or program. The information provided will be used only for class administration and related communications. If the requested information is not provided, registration or participation may not be completed. Personal data may be shared with authorized Utah County departments or contracted agencies involved in providing the class.
