Immigration Exams

The Immigration physical exam must be done by a doctor who is authorized by the U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (also known as civil surgeons) to perform the medical exam required for most Green Card applicants. Our clinic provides this exam to our county residents.UCHD’s Medical Director is approved by the U.S. Immigration and Naturalization Services to perform this medical examination.

The immigration physical exam process requires two (2) appointments and takes approximately one (1) to two (2) weeks for completion.

As of October 1, 2021, the USCIS is requiring all people applying for adjustment of status for lawful permanent residence (LPR) in the United States to have the COVID-19 vaccine. To find out where you can get a COVID-19 vaccine in Utah County please click here.

Bring with you to your appointment:

  • Current ID (government issued photo ID like a passport, driver’s license, employment authorization card, consular ID card, etc.)
  • Immunization/vaccine record
    • for those ages 6 months and older:
      • COVID-19 Vaccine. On September 12, 2023, ACIP recommended vaccination with the updated (2023–2024 Formula) COVID-19 vaccine for all persons aged ≥6 months.
        • Infants and children aged 6 months–4 years are recommended to receive a multi-dose initial series (previously referred to as the primary series) and at least 1 updated mRNA COVID-19 vaccine dose depending on vaccination history as defined herein, please see link here.
        • Persons aged ≥5 years without immunocompromise are recommended to receive 1 updated COVID-19 vaccine dose, irrespective of previous COVID-19 vaccination history. Please see link here.
    • for those ages 19 and over:
      • MMR (only if born in 1957 and after)
      • Varicella
      • Tdap
      • Flu
      • Polio
      • Hepatitis B
    • for those age 65 and over additional vaccine:
      • Pneumococcal
  • Insurance card ( for needed vaccinations only, immigration exam does not accept health insurance)
  • Method of Payment (cash or credit card, HSA card, we do not accept American Express)
  • Translator, if the applicant does not speak or read English.
  • Come well hydrated and do not urinate for an hour before your first appointment.

Physical examinations for United States Citizenship and Immigration Services (USCIS) form I-693 are provided by our civil surgeons Dr. Joseph K. Miner and Dr. David C. Flinders.

The I-693 form will be completed and placed in a sealed envelope for delivery to the USCIS office.

Lab/Blood Tests

The Wellness Clinic at the Utah County Health Department offers several laboratory tests for a discounted cash price.

Some of these tests may require for you to have a medical provider’s order, and at least that you have a medical home.

These services are offered as a Walk-in service during our clinic hours, except for STI testing and the QuantiFERON Tuberculosis blood test. To schedule a QuantiFERON TB blood test, please call 801-851-7022. For any testing related to sexually transmitted diseases, call 801-851-7538 to make an appointment.

Our hours are Monday thru Friday, from 8:00 AM – 12:00 PM and from 1:00 PM – 4:00 PM. To make an appointment or to make sure your walk-in service can accommodate you, you can call 801-851-7031.

Free Blood Pressure checks are available anytime during business hours of Monday thru Friday, 8 am – 4:00 pm.
